1.1 Who we are: These Terms and Conditions are for the services provided by Urban Wellness Health & Float (UWHF) 6 Balgownie Drive, Aberdeen, AB22 8FD a limited registered company in Scotland. Registration number SC564706.

1.3 How we may contact you. If we have to contact you we will do so by telephone or by writing to you at the email address or postal address you provided to us in your booking.
1.4 “Writing” includes emails. When we use the words “writing” or “written” in these terms, this includes emails.
Urban Wellness Health & Float Contract with you.
2.1 How we will accept your order. Our acceptance of your appointment will take place when we email you to accept and confirm, at which point a contract will come into existence between you and us.
2.2 If we cannot accept your appointment. If we are unable to accept, we will inform you of this and will not charge you (or will refund you for any payments made). This might be because of unexpected limits on our resources which we could not reasonably plan for or because we have identified an error in the price or description of services you have specified.
2.3 We only provide services in the Grampian area of Scotland. Our website is solely for the promotion of our services in this area.
Providing the services
3.1 One-off services. We will provide the services on the date and at the time of your appointment.
3.2 We are not responsible for delays outside our control. If our supply of the services is delayed by an event outside our control then we will contact you as soon as possible to let you know and we will take steps to minimise the effect of the delay. Provided we do this we will not be liable for delays caused by the event, but if there is a risk of substantial delay you may contact us to end the contract and receive a refund for any services you have paid for but not received.
3.3 If you do not attend the appointment. If you do not attend your appointment in order for us to perform the services as arranged, we cannot guarantee that we will be able to reschedule your appointment and you will not be entitled to a refund of the booking fee as this is a genuine estimate of the clinical time and product ordered specifically for client, which will we will lose as a result of your failure to attend on time/ at all.
3.4 If you arrive 10-15 minutes over your allotted time, it is very unlikely we will be able to accommodate your appointment and in this instance will result in a non refundable 100% deposit. Full allotted time is required to ensure a safe consultation and procedure is undertaken. Please call ahead if you think you may be late, as it may be the case that no other clients are booked in after you. We will do our best, however, you must understand that we do not wish to not risk safety or quality of care to our clients or staff from being rushed to fit someone in. It will be the practitioners call, and this will only be done if time allows.
3.5 Reasons we may delay the supply of services to you. We may have to delay the supply of a services to: (a) deal with technical problems or make minor technical changes; or (b) amend the services to reflect changes in relevant laws and regulatory requirements;
Safety of our clients
4.1 Ensure that the treatment is undertaken by an appropriately trained Practitioner.
4.2 Ensure that staff possess the applicable qualifications and registrations required for him/her to practice.
4.3 We will determine the suitability of treatment for you and provide you with the necessary information required to enable you to make an informed decision.
4.4 Work in accordance to the NMC, The Code.

Your rights to end the contract with us.
5.1 If you have just changed your mind about the services. Although you may be entitled to a refund if you change your mind about the services within 14 days of your receipt of our confirmation of your order, you will only be entitled to a refund of the full booking fee if we receive your notification of cancellation of the appointment no later than 72 hours before the appointment is due, as the booking fee is a genuine estimate of the clinical time and stock which will we will lose as a result of your cancellation.
5.2 When you don’t have the right to change your mind. You do not have a right to change your mind in once the services have been completed, even if the cancellation period is still running.
5.3 Ending the contract where we are not at fault and there is no right to change your mind. Even if we are not at fault and you do not have a right to change your mind, you can still end the contract before it is completed, but you will not be entitled to a refund of the booking fee as the booking fee is a genuine estimate of the clinical time and stock which we will lose as a result of your cancellation. If you want to end a contract before it is completed where we are not at fault and you have not changed your mind, just contact us to let us know. The contract will end immediately.
Your Commitment
6.1 You must advise us of all relevant medical information in advance of each appointment. UWHF will not be responsible for any incident that occurs as a result of failure to disclose this vital information.
6.2 You must adhere to all directives given to you pre and post treatment in regards of preparation and care.
6.3 You must be 18+ years for any intramuscular injections to be undertaken. If you are under 16 and have consent with a medical letter from your GP, you may be considered, however a consultation will be required to take place prior to consent of an appointment.
ADDITIONAL INFORMATION

Please arrive no earlier than 5 minutes before your appointment. We have no waiting area at the moment and you may have to stand outside longer than necessary until the practitioner or a member of staff can collect you from the door. The main entrance is locked at all times, please press the intercom bell, then allow us time to get to the door. Please be patient, we may be with other clients.

Full payment will be required to secure an appointment.

Any monies not redeemed within 12 months after the date of payment will be forfeited.

All appointment packages must be paid for in full at the time of the initial booking.

All packages must be used within the stated time on booking system. This may be 3 or 6 months of the initial payment. Where a medical need requires a package to be completed over a longer timeframe then the package must be completed within 12 months of purchase date.

Packages are non-refundable.

Refunds will only be given for medical reasons ( GP letter must be submitted) and will be calculated as full priced appointments, minus the amount used from the package. All package discounts or offer percentages will not be taken into consideration when calculating a refund.

Refunds will be made to the original payment card.

Products are non-refundable once opened.

Refunds will be made within 14 days of request.
Complaints
We endeavours to provide the highest possible service to you when delivering a consultation or treatment. Should you not be entirely satisfied with any aspect of the service or care provided, please contact us at you earliest convenience and we will do our utmost to resolve your concerns.
Confidentiality and Data Protection
We (UWHF) holds data relating to Clients in connection with the services provided to them in relation with these Terms and Conditions.
UWHF wishes to disclose data relating to Clients to appropriate staff in charge of their care and prior to their appointment . The Clients’ consent to such disclosure is deemed essential to their appointment.
The Client is expected to consent to the disclosure of sensitive personal data by UWHF to Staff for the purpose of discussing the Clients appointment.
It is acknowledged that UWHF is obtaining this consent for itself in order that they may comply with the requirements of the Data Protection Act 1998. Other than these disclosures or as required by a court of competent jurisdiction, UWHF will not divulge Clients’ sensitive personal data to third parties.
Our Liability

We (UWHF) will not be liable for any damage, accident, injury or death resulting from your (The client) failure to declare all relevant medical information.

UWHF are not liable in contract, tort or otherwise for any financial loss (including without limitation, loss of profit) or for any other special, Indirect, or consequential loss or damage arising out of, or in association with, its provision of any goods and/or services to you.
